I understand that dance and movement classes are physical activities that carry inherent risks, including the risk of injury. I am voluntarily enrolling my child, and I accept those risks on their behalf.
I, the undersigned parent or guardian, agree to assume full responsibility for any risks of injury, loss, or damage to my child that may occur as a result of their participation in this dance class. I acknowledge that Nicole Kadar, sole proprietor (doing business as Class with Ms. Nicole), is not responsible for injuries or damages resulting from accidents, negligence, or any other cause, to the fullest extent permitted by law.
I confirm my child is in good health and able to take part in physical activity, and I will let Nicole know of any condition, allergy, or special need relevant to their safe participation.
Photo consent is optional and captured separately at registration. If given, I consent to Nicole using my child's photos for marketing only when their face is not recognizable; Nicole will never use a child's name in a post. Photo consent is valid for 12 months and is re-confirmed each year.
I confirm I am the parent or legal guardian, that I have read and understood this agreement, and that completing registration and payment serves as my electronic signature and acceptance of these terms.
